To help out the staff i will make a format for easy veiw of players name and "Wrong-doing".

Players name: "player that has broken a rule or multiple rules"
Offince: "put what the player has done do get reported for"
Date: **/**/** "fill in so staff can see how long the ago the rule was broke"
Proof: "use a video/screen shot to prove this person has done wrong"
========================================================
If you do not have proof it is up to the staff on the punishment if any at all.
